1. Form 9331
2. Criminal History Report
3. Money Order for $35.00 payable to"State  of Indiana"
4. Transcript of 6 cr. Hours (3 crs. must be from VU)
5. Current License (with Internship requirement filled out on back if Required)
6. Send all paper work to:
