Please be aware that all SGLI students are require to have and show proof of health insurance. We cannot accept any student without health insurance. Also, please note that any student who has not completed and submitted their forms at least two weeks before your summer program begins will be dropped from the program. There are no exceptions to this policy. 

For American Students Only:

The following form is optional, and as needed. This form is to be completed by American citizens only. Not for International students.
