Immunization Requirements & Information

Current Immunizations/Testing Requirements for students:

1)  M.M.R. (Measles, Mumps, Rubella)-Two doses required at least 28 days apart for students born after 1956.

2)  Tetanus/Diphtheria (Td) or Tetanus/Diphtheria/Pertussis (Tdap)-done within the last 10 years.

3)  Tuberculosis (TB) The Testing section of the Health Form must be filled out and signed if you have lived or had extensive travel in haigh prevalence country, including Africa, Asia, Philippines, Indonesia, Eastern Europe, Latin America, Mexico, Portugal, Caribbena, and the Middle East.


Meningococcal - One dose for college freshman living in dormitories/residence halls, persons with asplenia, travelers to endemic areas, non-freshman college students who wish to reduce their risk of meningococcal disease.

Polio - Primary series, doses started at 28 days apart.  Three primary series acceptable.

Hepatitis A

Hepatitis B

Varicella- Birth in the U.S. before 1980, a history of chiken pox, a positive varicella antibody, or two doses of vaccine meet the requirement.
