Immunization Requirements & Information

Current Immunizations/Testing Requirements for students:

1)  M.M.R. (Measles, Mumps, Rubella)-Two doses required at least 28 days apart for students born after 1956.

2)  Tetanus/Diphtheria (Td) or Tetanus/Diphtheria/Pertussis (Tdap)-done within the last 10 years.

3)  Tuberculosis (TB) Screening/Testing section of the Health Form must be filled out and signed.  If the answer is YES to any of the questions, Valparaiso University requires that a health care provider complete a tuberculosis risk assessment (to be completed 6 months prior to the start of classes).

Recommended

Meningococcal - One dose for college freshman living in dormitories/residence halls, persons with asplenia, travelers to endemic areas, non-freshman college students who wish to reduce their risk of meningococcal disease.

Polio - Primary series, doses started at 28 days apart.  Three primary series acceptable.

Hepatitis A

Hepatitis B

Varicella- Birth in the U.S. before 1980, a history of chiken pox, a positive varicella antibody, or two doses of vaccine meet the requirement.
