2014 Personal Wellness Profiles

Employees who enjoyed discounted Health Insurance in 2014 must take part in the Personal Wellness Profiles to continue discounted insurance premiums in 2015.  To receive the discounted premiums, you and your spouse (if listed as a dependent on your insurance) must complete both the biometric screening and the online questionnaire.

Registration is now open for the Fall 2014 PWP’s.

Please go to the Optum Health Secure Registration Site and click on “Onsite Program” to schedule your appointment. Appointments are on a first come-first serve basis.  No additional dates will be added.

At-Home Test Kits 

We are again offering at-home testing kits as an alternative to the PWP for those who are not able to attend one of the on-site screenings. This test kit will help you obtain the information you need to evaluate your current health status, and track your progress toward maintaining or improving your health. As with the on-site screenings, Valpo only receives the names of those who complete the screening, not any individually identifiable information or individual results.

Employees and spouses will be able to order an at-home kit online using the same Optum Health Secure Registration site
.Those intending to utilize this option need to have kits ordered by 5 p.m., Friday, Nov 14

The kit will arrive at your home in 3-5 business days, and it needs to be completed and returned to Optum in the provided postage paid envelope
.  You will receive an email when your results are available for you to view via the account you create on the site. Optum has produced a short video to demonstrate how the at-home process works.

Health Assessment Questionnaire

As in previous years, you must complete both the biometric screening and the online questionnaire to be eligible for the discounted insurance rate.  The questionnaire may be accessed on myuhc.com, and must be completed by midnight, November 30, 2014.

Please refer to this instruction sheet to guide you through accessing the questionnaire.  If you have problems with the website, you need to contact UHC at 877-844-4999.

Please be sure to note the following information:

  • You may complete the questionnaire prior to receiving your screening results (this will not affect your results). 
  • If your spouse is covered by your insurance, you and your spouse must have separate log-ins to complete the questionnaire.  Your spouse (if currently on your insurance) can create a separate myuhc.com login using the same insurance member number.
  • If you are not currently on Valpo insurance, but will be in 2015, you will not have access to myuhc.com until 1/1/15.  Please contact Paula Shreve (x5092) for special instructions.
