Printing and Returning your forms
After printing your form, complete it, obtain the appropriate signatures, and return it to the Office of International Programs.

Employment
On-Campus Employment
All employment must be authorized by the Office of International Programs before you begin working.
Curricular Practical Training (CPT)
For information on employment during academic program/before graduation.
Optional Practical Training (OPT)
For information on employment pre- and post-graduation.
Medical Insurance
Medical Insurance Claim Form Medical Insurance Policy 08-09 Extension of Stay
If you are unable to complete your program by the date indicated in item #3 of the DS-2019 form or Item #5 of your I-20 form, you are required to apply for an extension. You must do this BEFORE your DS-2019 or I-20 expires!
Reduced Course Load (RCL)
Every international student holding an F-1 or J-1 visa is required by immigration law to maintain a full course of study every semester. Full time is defined as 12 credit hours per semester (9 credit hours for Graduate students). If you are unable to meet these requirements, you must seek prior approval from the Office of International Programs before dropping any courses. Failure to seek prior approval may result in a violation of your immigration status.
