Step 2 - Submit Health Form and Insurance Waiver
In accordance with United States federal law mandated by the Affordable Care Act, all adults living in the United States must have proof of health insurance.  Therefore, as a new student coming to live in the U.S., you are required to provide proof of your health insurance.  

  • Everyone must complete this Health Form and Insurance Waiver.  If you have your own health insurance, the policy must meet the requirements listed on the form.  
  • The form is due by the first day of classes.  If the Health Center does not receive your Health Form and Insurance Waiver by the first day of classes, you will be automatically signed up and charged for VU insurance, no exceptions.
