International Student Health Form
Make sure you fill out the Valparaiso University Health Form, paying particular attention to the section on required immunizations, and return it as soon as possible. If you are not in compliance with the immunization regulations, you must obtain the immunizations at your own expense at Valparaiso University before you can register for classes.
Valparaiso’s International Student Health Insurance:
At Valparaiso, according to the new national healthcare law, you
are legally required to have health insurance while studying in the
United States. Your insurance must also meet the University's

insurance coverage by Wednesday, January 7th, 2015, the first day of classes , or before that day by email, sending your coverage to firstname.lastname@example.org .

- Medical benefits of at least $500,000 aggregate medical maximum per policy lifetime..
- Repatriation of remains in the amount of $7,500.
- Expenses associated with medical evacuation of the exchange visitor to his or her home country in the amount of $10,000.
- A deductible not to exceed $500 per accident or illness.
If you have medical coverage from your country or sponsor, then you
must show VU proof of that coverage.
If you do not show proof of your coverage before or on January 7th, the first day of classes, VU will automatically
sign you up for Valparaiso University's insurance. If you want this
insurance, please advise us. It will cost you approximately $860.00 for 7 months of insurance until August 2015.
