A fast, convenient, and affordable way to have your passport photos taken.


What to wear for the photos:
  • Normal street attire
  • Hat or headgear that obscure the hairline and uniforms should not be worn unless it is religious attire that is worn daily. A signed statement must be submitted with the application verifying the item is worn daily for religious reasons. However, in no case should the item or attire cover or otherwise obscure any part of the face.
  • If you normally wear prescription glasses, a hearing aid, wig, or similar articles they should be worn for the picture
  • Dark glasses or nonprescription with tinted lenses are not acceptable unless you need them for medical reasons (in which case a medical certificate may be required).

Students and Staff receive two 2"x2" photos for $10 with a valid Valpo OneCard ID

All others pay only $12


IMPORTANT: The passport service in the OneCard Office ONLY prints the government specified photos necessary for a passport application; it DOES NOT complete any other part of the application process.

Apply For Your Passport!


You must apply in person if:

  • You are applying for your first U.S. passport
  • You are under age 16
  • Your previous U.S. passport was issued when you were under age 16
  • Your previous U.S. passport was lost, stolen, or damaged
  • Your previous U.S. passport was issued more than 15 years ago
  • Your name has changed since your U.S. passport was issued and you are unable to legally document your name change

The following information is for first time applicants over the age of 16. If this is not you, visit http://travel.state.gov/passport for more specific information or any other questions regarding the application process.


  • Complete Form DS-11: Application for a U.S. Passport
    1. Complete the application online before printing it out
    2. Print out a blank form, then fill it out
    3. Obtaining one from an Acceptance Facility

THEN submit the form and additional documentation found below at an Acceptance Facility.
NOTE: Do NOT sign the application until instructed to do so by the Acceptance Agent.

  • Additional documentation:
    1. Identification must be presented at the time of application (one of the following):
      • Previously issued, undamaged passport
      • Naturalization Certificate
      • Current Government ID (city, state, or federal)
      • Current Military ID
    2. Application Fee for your passport book:
      • $75 Application fee (made payable to “Department of State”
      • $25 Execution fee (paid separately to the facility where you’re applying)
    3. Evidence of Citizenship (one of the following):
      • Previously issued, undamaged passport
      • Certified birth certificate (Native Indiana residents can get certified birth certificates from the Indiana State Department of Health. More information can be found at:

      • Consular Report of Birth Abroad or Certification of Birth
      • Naturalization Certificate
      • Certification of Citizenship

NOTE: When applying for a U.S. passport in person, evidence of U.S. citizenship must be submitted with Form DS-11. All documentation submitted as citizenship evidence will be returned to you by mail.
