Important Notice: As of August 15, 2005, all usable computers are committed to the equipment replacment and upgrade program, which seeks to replace most Pentium III 500 MHz and below computers with "something better."

 

Consequently, recent-model computers are not available for reallocation.

If the machine being replaced is not available at the stated location at the time the new equipment is to be installed, IT staff will return the new equipment to storage pending resolution of the discrepancy. If the machine being replaced is serviceable, IT will reallocate it based on pending requests and Decanal or Vice Presidential priorities.

This form should mail itself to the Requestor E-Mail Address which you provided above. Please recheck that address.

When you receive the confirmation copy of the form, forward it to your Department Chair (or Director), who will forward it to the Dean (or Vice President), who will forward it to IT.Office@valpo.edu. These endorsements are required before IT can honor the request. In addition, further administrative endorsement may be required, particularly if the amount requested from general funds exceeds the amount allocated for the stated purpose.
