Grant Proposal Activity Form (GPAF)

The Grant Proposal Activity Form must be fully approved before any proposal can be submitted.  It includes all basic proposal information including investigator information, budget figures, and project and funding agency details.


Conflict of Interest Disclosure Statement Form (DSF)

The Conflict of Interest Disclosure Statement Form is required before any proposal is submitted to the funding agency and ensures that potential conflicts of interest are captured at the proposal stage.


Prior Approval Form

The Prior Approval Form is required to be submitted to OSR prior to any no cost extension, budget revision, or preaward costs can be approved.
