PRODUCTIONS REQUEST FORM

Note: Please submit request no later than two weeks prior to your event. Requests submitted less than two weeks prior to the event may be subject to a $25 or 10% late fee, whichever is greater.

**EFFECTIVE JANUARY 2007, PRE-PAYMENT WILL BE REQUIRED FOR PRODUCTION SERVICES.**
You will be sent an estimate and invoice via email shortly after your request is approved. If you labor charge turns out to be lower than invoiced, a refund will be issued immediately after your event. If the labor is higher, you will be billed the difference. If we do not receive payment or proof of a budget transfer two days before your event, we will not be able to provide services. If this is an issue, please contact us immediately so we can work with you.
