Health Form and TB Questionnaire

Valparaiso University requires every student to receive and submit Indiana State required immunization as well as create a health center portal to complete health forms.

The student will receive an email invitation to the Health Center Patient Portal via the student’s Valpo email after the student deposits or confirmation to attend Valparaiso University.

All health requirements are reviewed by the health center staff, who check the immunization dates for compliance with state and federal regulations.

Due to privacy regulations, all communication regarding the student’s health information must be with the student.

These health requirements are due in July for all summer and fall semester students and in December for all spring semester students.

If the Health Center does not receive the student’s information or the information is incomplete, the student will be charged $50 (payable to the Health Center only) and a hold will be placed on the student’s account, making him/her unable to obtain transcripts or register for classes for the following semester until requirements are met.
